EOL/Medical Aid in Dying (MAID) Advocate
Providing the practical and profound support at end-of-life.
Service Description
-Provide education and support around end of life options; with speciality in Medical Aid in Dying (MAID). -Discuss/Review End of Life (EOL) Care Plan Logistics. -MAID process coordination, ensuring documentation is in place. -One In-home Care Plan Visit or Zoom (calls, email, text included) -Liaison with hospice and other team members (family, friends, medical teams etc…) -On hand support day of taking MAID medication (ie-prepping medication, coordination with hospice, other team members). -Ensuring clients end of life wishes above medical needs are honored. -To be a support, guide and resource for clients (person who is terminally ill and family) emotionally, spiritually and practically, pre, during and post death.
